LB-456088-2 CLINICAL IMPACT OF A UNIVERSAL REMOTE MONITORING PLATFORM FOR ICD AND CRT-D FOLLOW-UP FROM A LARGE REAL-WORLD REGISTRY

چکیده
Remote monitoring (RM) is standard of care for patients with cardiac implantable electronic devices (CIEDs). Questions remain regarding data management and whether RM affects clinical outcomes. This study aims to evaluate the real-world impact conventional vendors’ solutions versus a universal platform (ImplicityTM), using French national health database. Medico-administrative on patients’ characteristics, hospitalizations deaths in population across 10 years were collected. All ICD (including CRT-D) considered labelled depending type follow-up (all-RM, no-RM, ImplicityTM RM, RM) during year 2019. Patients change modality excluded. We analyzed outcomes (all-cause mortality, number cumulative duration all-cause hospitalizations) according (all-RM vs RM), different statistical methods, after considering potential confounders (age, gender, CIED type, implant). Study cohort consisted 76,418 CRT-D 24,115; age 67.6 ± 13.6 / male 78.1%). All-RM had better survival compared no-RM group (-9%, p=0.0012, Fig. 1 Left), an increased (12%, p<10-4) (7%, p<10-4). Difference remained significant mortality (-10%), was eliminated (2%) inverted (-6%) adjustment (Tab. 1). Compared associated decrease (-28%, p<10-4, Right), (-7%, p=0.0244) (-13%, p=0.0015). (-26%), similar (-7%) (-13%) Higher risk patients, identified as those chronic HF diagnosis or prior HF-related hospitalization, superior (+28%), reduction (-12%) (Tab.1). remote preliminary focused population. Further work will assess outcome pacemaker CRT-P population, extended duration. Tabled 1Comparison modalities rate, differences correction doubly robust model. Follow-up of CRT-ICD: implications for the use of remote follow-up systems. Data from the InSync ICD Italian Registry.
BACKGROUND Launch of remote follow-up systems in Europe is currently underway. However, there is insufficient understanding of postimplant practices with respect to device follow-up, reprogramming of device features, and postshock clinic visits. METHODS We analyzed device-stored data from patients implanted with biventricular defibrillators (CRT-ICD) to characterize the management of patients...
متن کاملClinical decision tool for CRT-P vs. CRT-D implantation: Findings from PROSE-ICD
BACKGROUND Cardiac resynchronization therapy (CRT) devices reduce mortality through pacing-induced cardiac resynchronization and implantable cardioverter defibrillator (ICD) therapy for ventricular arrhythmias (VAs). Whether certain factors can predict if patients will benefit more from implantation of CRT pacemakers (CRT-P) or CRT defibrillators (CRT-D) remains unclear. METHODS AND RESULTS W...
متن کاملLong-term outcome after ICD and CRT implantation and influence of remote device follow-up: the ALTITUDE survival study.
BACKGROUND Outcome data for patients receiving implantable cardioverter-defibrillator (ICD) and cardiac resynchronization therapy-defibrillator (CRT-D) devices treated outside of clinical trials are lacking. No clinical trial has evaluated mortality after device implantation or after shock therapy in large numbers of patients with implanted devices that regularly transmit device data over a net...

The role of cardiac resynchronization therapy (CRT) in patients presenting with mild manifestations of heart failure (HF), depressed left ventricular ejection fraction (LV EF), and wide QRS complex, has been addressed in four previous trials: MIRACLE ICD II,(1) MADIT-CRT,(2) RAFT,(3) and REVERSE.(4) The consistent observed benefits in reverse cardiac remodelling and reduction of heart failure a...
متن کاملSpinopelvic Fixation of Sacroiliac Joint Fractures and Fracture-Dislocations: A Clinical 8 Years Follow-Up Study
Background: Pelvic ring injuries and sacroiliac dislocations have significant impacts on patient’s quality of life. Several techniques have been described for posterior pelvic fixation. The current study has been designed to evaluate the spinopelvic method of fixation for sacroiliac fractures and fracture-dislocations. Methods: Between January 2006 and December 2014, 14 patients with sacroi...
